SpamAssassin is a Perl module, with some stand-alone programs to scan an
email.  ClamAV is a stand-alone program (or a Perl module) that scans
files, also.  MailScanner works with the MTA (Exim, Postfix, or Sendmail)
to provide a coherent way to apply processes such as SpamAssassin and
ClamAV to mail messages and use their output to mark the messages.

MailScanner is well-documented in the config file, and you'll also need
to read a specific tutorial for setting up your particular MTA to use
it.  It's not difficult.
