Is this with kde?
If so, sometimes kcheckpass gets it's permissions messed up.
Look in /usr/lib64/kde4/libexec/kcheckpass or equivalent location for
your distro (I'm using gentoo).
Test it by running it from a shell. If it doesn't return 0 or provides
some other error that may be the problem.
