Looks like mplayer is working but the codec can crash? Does this crash occur if you leave the manually started mplayer going?
If you start the server from the command line and capture the output of --d_source and mplayer when you try to play something you may be able to detect a problem. If you edit slimserver-convert.conf to change the #PIPE# to #DEBUGPIPE# you will get more debugging info which may help.
