You should be able to load the plugin from
http://downloads.sourceforge.net/project/lms-to-upnp/dev/repo-sf.xml -
that's where mine comes from.  You can test it by just putting that in a
browser as an address and see it download the xml file.  It really
should show up in the plugin list (near the bottom) provided you have
allowed 3rd party repos (at least, that used to be the case), and
(possibly) restarted LMS.

I used to run it stand-alone, but it's a wee bit of an exercise to get
working automatically.  I ended up using the "Server Power Control"
plugin autoexec command facility to run a script to start the plugin.  I
found I had to make sure LMS had started before running the external
program, and this plugin's command allowed me to do that.  

There are other ways of course.  You can make sure that the LMS startup
script creates some sort of variable that your UPnP service startup
script depends on.  I'm on ubuntu, and the service startup methods have
gone through several iterations, which caused me to do quite a lot of
research before getting a working solution.

renderers.  
Squeezelite to Meridian USB Explorer DAC to PC speakers/headphones.  
Wireless Xubuntu 16.04 laptop firefox/upplay or Android 'phone with
Squeeze-Commander/BubbleUPnP controls LMS/Minimserver.  
Also a Touch with EDO, plus spare, unused.
