Are you using MusicIP (MusicMagic)? If so the following bug might be the cause: http://bugs.slimdevices.com/show_bug.cgi?id=3824
There's a partial fix for that one that's in the current nightlies (from a couple of days ago), but there appear to be some residual problems remaining.
