Yes we are talking about different things. Icons can typically show up in three instances: Menus , favorites and when playing. Often Favorite icons don't show up when playing - I thought this was the case.
The icon in menus driven by the install.xml and so should display normally as long icon is in the location and the HTML settings match the installation (i.e. EN is for English) - although I think EN is the default if there are no other languages defined.
