Under Apps > Spotty > Playlists I see the playlists I made using the
Spotify app. Under My Music > Playlists I see the same playlists, with
"Spotify:" prepended.
Now, when Spotty plays a song from Spotify, I can right click on the
song in the web interface, select Add to playlist, and select one of the
"Spotify: <name>" playlists.
However, the new song does not appear in the selected Spotify playlist.
Instead a new playlist "Spotify <name>" (without the colon) is created
under My Music > Playlists, with only that song in it.

Oh, that's an interesting question. And actually surprising it hasn't come up before. Here's what you're seeing: the Spotty menu presents you the data as stored on Spotify. Whenever you navigate there, the plugin requests the latest information from Spotify (ok, some caching is involved, but it's only a few minutes for personal content).

The My Music / Playlist menu on the other hand is fed by the music scanner's database. It imports all content from Spotify, too, but keeps references in the local database to merge it with your local music. These references aren't updated "real time". But LMS is polling the service for changes every now and then (currently once an hour). Which means that that list would eventually update, but it can take an hour before this happens.

When you add a track to a playlist from the Spotify menu, you change the Spotify playlist, not its local copy.
