Hm, I'd suspect there's either no update of the status after skipping the track or the old status query (the one documented in the CLI spec) which iPeng uses doesn't update the artwork.
iPeng never caches NowPlaying artwork (OK, it does now for 4 images on iPad but not on iPhone) but caching would not cause such an issue. Caching caches images for a certain track ID, so if you are seeing a wrong image, something's wrong with the track ID.
