Dejan, I typically use ArcView to create the reference map image and return the extent of this image. The reference map does not need to have the same extent as the main map, it's just necessary that the extent of the refmap image is really correct and in the same coordinate system as the main map. One can eg. choose a bit bigger extent for the refmap than for the main map so that the red position rectangle is always visible.
Maybe some tools from maptools.org provide functions to deal with that, like chameleon or maplab. There was also a tool for managing map files on sourceforge, perhaps it supports also the creation of reference maps and returns its extent... You could use also pmapper 2 and resize the window until you reach the size for the main map that you like to have for the refmap. Then after the zoom/pan retrieve the extent from the session variable GEOEXT. Not very elegenat but it should work. I can provide you a script that can display a bit better structured the session variable contents, also for some debugging not so bad.
