When a player first sits at the table and is not on the big blind, he
is authorized to enter the hand if he posts the big blind. I assume
this is what happens here. The situation where the big blind is posted
with a small blind (dead) is when an existing player was sitout and
missed the big + small blind. If this player wants to enter the next
hand, he will have to pay both the big and the small blind.
Alternatively he can wait for the next big blind and just pay the big
blind.
