We also have two servers with identical setup (postfix, policyd and
MySQL). Both machines have 2 IP addresses: one system has 192.168.0.10
(host1) + 192.168.0.11 (smtp1) and the other system *.12 (host2) + *.13
(smtp2). 

If one system goes down (say, host1/smtp1), then the heartbeat service
will automatically configure the smtp1 IP address on the machine that is
still alive (host2 now has smtp1 and smtp2 (*.10, *.11, *.13)). If the
original machine comes back again (on the host1 IP), the heartbeat will
see that and will release the smtp1 address and all goes back to normal.

We do not replicate the database. Both servers are set with a reasonable
throttle limit. True, this doubles the possible spam rate, but at least
you have a totally redundant and independent setup where the service
will be available to users almost continuously.
