2) requests that hang and don't time out
3) lsof did show what seemed to me like excessive number of fds.

On the occasion I looked at fds I had browsed to kernel.org and that site uses
https exclusively so maybe some tunnel issue??

I also notice that polipo logs tunnel messages for sites that are
unreachable. Maybe that's a feature, I don't know. I would check to see
if a site's reachable before I do anything else with a downstream
request for upstream resources.

Sometimes a restart would help, often I'd remove the cache as well.
But I don't use it anymore and the version in use now is working.
