The premise is wrong. In HSS 303 the requirements for info to be placed on the COLB (which is NOT a legal document for claiming "natural status" for state services to "natives") are birth "registration" (NOT live birth) in Hawaii.
This is why ONLY the vault copy will do. It requires and will list the doctors signature and affadavit of birth.
