update www/c-icap/modules to 0.5.7 and merge subpackages (c-icap-clamav
and c-icap-urlcheck) into c-icap-modules. clamav_mod is working for me,
clamd_mod is not, but it wasn't working in 0.5.6 either (squidclamav should
be working for a clamd-based scanner).
switch www/c-icap/c-icap to github upstream, but kept at 0.5.13 for now,
0.6.3 is just segfaulting at startup for me.
