As milter is only used when receiving mail by postfix, not much as mail can have multiple recipients, local and remote.

I was thinking about encoding the forward-email addr into
[email protected] in database and using special
transport for this domain and having SRS enabled only for this
transport (and then rewriting it back to user@gmail using PCRE
aliases). Would it work? Is it over-complicated?

I don't fully understand how you mean that.
But I guess it would become more complicated than just using multi instance.
