I didn't have that ticked just now ('Sort by caption'), so turned it on, 
keeping the specified sorting order in its field still, and now things 
seem to be working...  I thought I'd tried that combo before, but 
perhaps not!

Yep, had tried that one & it didn't seem to make any difference.

The sorting order working properly is a nice improvement though --
