That was a Global Village modem--they were somewhat popular with Apple
IIGS folk too, tho some people found that their ADB port (where the modem
plugged in) didn't always have enough "oomph" to power both the modem and
the mouse.  The ADB plug had a piggyback connector as a pass-thru to let
you connect your mouse/keyboard.

I don't have one to give the model # but I did use one for a short period
of time.  I think it was a 14.4--I don't recall it being 28.8 but I could
be wrong on that score.
