PVNGS requests the following discrete particle control and SDE recording
information:

1. Does your facility perform field monitoring for discrete particles on
personnel?
a. If so, at what threshold (e.g. - open window RO-2 reading of 60
mrem/hr) is a formal dose evaluation performed?
b. If so, are discrete particles below the threshold simply discarded or
is the event documented?
c. If so, what is the maximum time between monitoring?
d. If so, what threshold is used for entering the dose into your dose
tracking data base?
2. For personnel contamination events, what threshold is used for entering
the dose (SDE, WB or ME) into your dose tracking data base?
3. Does your facility report SDE from discrete particles in Box 13 (SDE,
WB) or Box 14 (SDE, ME) of the workers Form 5 with a comment in Box 19
(Comments) or only in Box 19.
