Subject: Powernet: Are Anticipated Dose Rate Alarms Documented in CAP?

Robinson would like to determine if other sites document "anticipated dose rate 
alarms" which occur after they have been discussed and documented during a 
pre-job brief, as "Adverse Conditions" using their site's Corrective Action 
Program (CAP).  At Robinson our procedure requires us to perform a documented 
"Direct Reading Dosimeter" investigation for all dose and dose rate alarms, and 
to also document all dose and all unanticipated dose rate alarms in CAP.

Does your site document anticipated dose rate alarms in CAP? No.  We use the 
Health Physics electronic shift log to document anticipated alarms.  All 
anticipated must be pre-briefed and documented in the RWP logbook prior to RCA 
entry. HP Supervision approval is required for all anticipated alarms.
Does your site document unanticipated dose rate alarms in CAP?  Yes. All 
unanticipated dose rate alarms as well as all dose alarms.
Does your site use another tracking system (besides CAP) for tracking all dose 
and dose rate alarms? Yes. Health Physics Instruction (HPI) procedure requires 
and electronic dosimeter alarm   investigation form to be completed for all 
alarms. These forms are retained in plant files and attached to the Condition 
Report.
