At GGNS there isno documentation for passive body counts using a PM-7 type of 
instrument.  Since we have them in security island and there is no way out of 
the plant (except for emergencies) without passing thru one we know they had a 
count if they went into our protected area.  Now given that, we still prefer 
they come by dosimetry and be observed.  We have a check out form that runs an 
exiting employee through their boss, dosimetry and security to 'ensure' they 
follow the process, and that includes initialing that they went through a 
passive monitor, but I am not deluded enough to think that everyone follows 
that process, even though it is proceduralized.

We have multiple examples of people just leaving site without checking out and 
people that go home and may not come back due to some issue.  We know they had 
a passive exam, it just wasn't observed by us.

Browns Ferry currently documents all exit passive monitoring counts for a 
terminating worker on a log and the technician observing the count signs the 
log.  Based on previous benchmarking I have always felt that the individual 
being counted should also sign to acknowledge the count.  That part was never 
implemented due to objections from our corporate office.

Now our corporate management has decided that we should no longer keep the log 
of passive counts for terminating workers that had been performed.  To me if a 
survey is not documented, it was not done.

If you use passive monitoring,  what documentation do you retain for a 
terminating worker?  Does anyone not keep a record of these counts or who 
observed them?  I have never been to another utility where they have not at 
least been observed when terminating.
