Palo Verde provides the following response:

Do you document high rad briefings on a written record? Yes.  At a minimum each 
briefing is documented in an electric log book, for more complex RWPs this 
briefing may also be documented on a written briefing sheet.

Do you have a worker sign that they have been briefed for a High Radiation 
area? Yes, this is accomplished thru an acknowledgement during the access 
control process.

If you do document the briefing, how long do you retain the record? Retained 
for life of the plant plus 10 years
