Are surveys required to be documented using a survey map? When are surveys in the narrative form acceptable? Is documenting a range of dose rates or contamination levels without noting the specific location acceptable? For example the general area dose rate in the room ranged for 2 mr/hr to 20 mr/hr without noting the specific locations of the lowest or highest dose rate. Who performs the final review of the survey prior to submittal to plant records - Technician, Supervisor, Manager, QC/QA?
