Exelon is conducting an informal benchmark of OE 36193 - Reactor Power
Change During Reactor Containment Building Bioshield Entry (Harris). The
OE lists the following lessons learned and corrective actions:

 

*       Work activity risk management processes must include both online
and outage work activities in plant reactor modes 1 and 2.

 

*       All Radiation Protection procedures that are implemented with
the plant condition of reactor at "power" will be implemented when the
reactor is "critical".

 

*       Prior to any entry into the bio-shield the Radiation Protection
Manager shall ensure there is alignment with Operations, Duty Manager,
and Radiation Protection that reactor power is planned to be constant
while personnel are inside the bio-shield and the reactor power level is
consistent with the conditions briefed.

 

*       For emergent and planned entries into the bioshield wall the
Duty Manager, Radiation Protection Manager, and Nuclear Shift Manager
must be briefed on the ALARA projections and the basis for the estimate
(that is, reactor power level).

 

*       IAW PLP-511, Radiation Control * Protection Program, all entries
inside the bio-shield wall, with the reactor critical will be planned
and executed as High Radiological Risk Significant Activity (RRSA).

 

Exelon would like to know: For BWRs, do you have any additional guidance
in steam dependent areas that would prevent another event similar to
this OE?
