* what is primary method of documenting survey information, paper or electronic survey program? Electronic, paper is allowed during outages for contractors. Paper surveys are then scanned and entered into the database. * * how does worker review survey information for job planning / prior to entering RCA, paper or electronic copies? Paper, we are transitioning to electronic but will still have paper available until the work force becomes familiar with the electronic maps. * * how (in simple terms) is survey information controlled so that only most current survey information is available for viewing? HP Technicians update the electronic display after completing their routine survey for each area. The displays are password protected. * * prior to entering RCA, are all workers required to interface with RP / HP personnel to validate radiological conditions for area(s) to be accessed? Survey maps are posted at the HP Access Control Point, workers are required to review the maps for radiological conditions in the areas they wish to access, fill out a TRIP ticket and check in with the HP Lead Technician prior to entering the RCA. The only exemptions to this process are for control room operators, chemistry lab workers and tool crib attendants. If a worker who is exempt from the RCA check-in process is required to perform tasks outside of the control room, chemistry lab or tool crib they are required to report to HP Access Control, complete a TRIP ticket and be briefed the HP Lead. * * if interface with RP/HP, how often (every entry, shiftly), what (if any) groups are exempted (Ops, Chem, Security)? The entire crew is required to check-in upon initial entry to the RCA, if returning to perform the same task only one individual is required to inform the HP Lead of the work crews return to the RCA. If the crew changes locations or starts another job the entire crew will complete a new TRIP ticket and be briefed by the Lead HP Technician. * * are trip / info tickets used for all RCA entries?
