For PSEG Nuclear:

Individuals with a signed Form 4 (or equivalent) will be issued a dosimeter.  
We have an in-house form that gives the individual an opportunity to provide 
their exposure information – and we validate that in PADS. The PADS information 
is reviewed with the individual and they sign electronically (attesting to the 
accuracy of the information).  By procedure, no dosimeter is supposed to be 
issued without validation of the current year exposure – there can technically 
be off-hours dosimetry issue, as our Shift Techs are trained to issue 
dosimetry, but do not have PADS access, so it would require the individual 
receiving the dosimeter to provide valid exposure information.  Although this 
is technically possible, it very rarely happens (exception being outages where 
staffing is different).  Any off-hours dosimeter issue would be validated by 
Dosimetry staff the next morning in PADS to ensure limits are appropriate.

Hello all,
Currently, at Cook we have incoming radworkers document and attest to their 
current year dose. We then validate this attestation with PADs immediately, 
before issuing the TLD (following WBC).

Does your site require that current year dose be validated on PADs prior to TLD 
issue?

Does your site allow issue of a TLD without validating current year dose beyond 
attestation w/ signature?.

If your RP Techs issue TLDs after hours, are the PADs qualified?
