Pushed some changes which hopefully addresses the remaining feedback, hopefully everything looks ok! I merged in recent changes from 4.1 and ran a [full CI run](https://issues.apache.org/jira/browse/CASSANDRA-19703?focusedCommentId=17950821&page=com.atlassian.jira.plugin.system.issuetabpanels:comment-tabpanel#comment-17950821).
