We are
basically treating these requests as "changes of address" for all services and
notating in our systems that the old address is not to be used. This was
the only way we could honor the patient's request while insuring that all
downstream parties had the informaiton. If the patient then decides to
change the address again, they will be notified that ALL mailings for past or
future services will be sent to the new address. We just couldn't
accomodate "per treatment" within our systems.
We
struggled with this, the intent, and the difference between Confidential
Communication and Privacy Restrictions. Privacy Restrictions, of course,
would be treated differently and per treatment if approved by our Privacy
Official.
This
was the best option we could offer our patients.

Would anyone be willing to share how their CE is handling the issue of confidential communications which may be requested under HIPAA by the patient. We are a large healthcare organization with some 300 plus computer systems that do not communicate with one another. We have considered placing parameters on the patient's request such as per "episode of care" and requiring the patient to make the request at the point of entry to the system -- that is the clinic, hospital, etc.. It would be helpful to hear what others are doing.
