The paper describes the obvious records...medical and financial.  It also
describes other catagories that are very broad such as "working records"
(telemedicine, coding worksheets, utilization review worksheets etc.), and
records/information collected on your behalf by a Business Associate and
health information in all types of media (paper, oral, video, etc...) when
such information is created or maintained for the purpose of making
decisions about individuals...

In my opinion, this sounds like the designated record set can be huge and we
had better know where all those little pieces of information reside in case
someone invokes their right of access.

I see us doing some serious centralization of information.

Leah -- I am not sure what type of a response you are expecting from anyone
but here it goes one example of how we are determing a designated record
set...

We are modifying our existing system to support the ability to view or print
a member's PHI that we use to make decisions regarding an individual for
utilization management.  For example, I use 9 out of the 18 individually
identifiable health information identifers to approve an authorizaton.
These 9 identifiers become our designated record set that we used to make a
decision about the member.

We are applying the same rule in any situation where we may make decisions
for a member.
