Thanks to everyone who re-sponded to my urgent request for help. In the end, I selected the screw head and simply used curves to lighten the shadows and then de-saturated to remove the colour noise. I am left with a suitably aged looking screw head which fits in perfectly with the hand-wrought curtain pole fittings. Sometimes it is the obvious idea that get overlooked!
