I had a similar problem with my Mamiya 645 pro (on film), well everything was out at max apertutres(!), I found that my mirror was out of alignment. Every thing was in focus even on a tri-pod. Don't know if this may be the case? There was a thread on this topic month or so ago I think. Check out the archives maybe. just an idea. I'm following this thread too as I shoot on a 14n but don't suffer these problems.
