When I first called about the ISV program I was told that indeed the MDSN licenses do not expire, but the updates do stop coming monthly at the end of the Subscription period. In the past I had a choice between CD and DVD media. My first year I did CD, the 2nd year I did DVD. The DVD media was far less bulky, but on older PCs that have no DVD drive it was a PITA. But, those older machines are rather rare around the "Gil Hale House That FoxPro Built". For the bulk reduction along I prefer the DVD media.
