Thanks for that reference.  My problem is that I can get the context menu to run PSPad normally or as administrator, it's just that both programs (PSPad.EXE and PSPadAdmin.EXE) appear in the context menu as "PSPad" with no way of distinguishing between them.  By the way, I believe that the open with choices are stored in the registry at

HKEY_CURRENT_USER\Software\Microsoft\Windows\CurrentVersion\Explorer\FileExts\.FPW\OpenWithList

Where .FPW can be any other file extension.
