I was going todismiss it as phishing, too, but as Pete says, you can 
call the number.  Since you can't find the card, the number on their 
contact page is:
*Customer Service and General Information*
1-800-528-4800 / collect Int'l 1-336-393-1111

FWIW, there IS a part on their web page which says:
_Patriot Act Notice:_ Federal law requires all financial institutions to 
obtain, verify and record information that identifies each person who 
opens an account, including your name, address, date of birth and other 
information that will allow us to verify your identity.

I would think that the fact that they reached you at your number, 
confirms your number, and they could confirm your other facts by reading 
what THEY have, and you saying, "uh huh."
