That also happened in a place where I worked. They let some flunky secretary run the backups and she neglected to tell up of error messages that were popping up. Luckily it didn't result in any loss.
This is why, ultimately, tape is bad. Backups must be done automatically without any human interaction for them to be effective. In all my clients I have 1. Machine to machine, multi-day backup 2.
