For my implementation, look at p129 in the first link for conversion
from (x,y) to (x,y,z).
Note, the (corrected) script from my notes are available

but using x. y. era notation and lots of x y local variable so updating
to modern J will require attention.
I will be interested in updating this to modern J. Perhaps a project for
next week. It has been 10 years since I last thought about this so I'm a
bit rusty.
