I'm assuming you are routing the vo cursor to the mouse prior to command clicking? does VO announce what's under? Are you command clicking fairly quickly. DO you have a large amount of tracks in your session? Sometimes when this happens to me, I save a preset of what I have in the plug, and switch to no insert and then reinstantiate the plug.
