Conor, I have worked with MUMPS/Cache based EHR where the local data dictionaries were duplicates of controlled vocabularies/ Ontology. This way mapping, updating and maintaining is much easier. I worked on ways to tag (annotate) the data end users capture (MD/RN) to these data dictionaries, at the time of data entry itself. This way we could capture research required data discretely and reduce the dependency on natural language processing to a certain extent.
