I've spent a wee while this morning with it and as far as I'm able to tell, <programlisting> appears always corrupted in the pdf.

<Screen>, on the other hand, has started to work! I duplicated my example:

<screen>
-----             <-- top window
           ---    <-- unlocked, but not top
 -------          <-- locked
-----------       <-- bottom window, also locked
</screen>

And in front of the first hyphen of the first line, I inserts a single non-alphunumeric character. One each for every character on my keyboard which is non-alphanumeric. Each and every one of them rendered correctly, including the original one! This implies that it must be a combination of things that cause <screen> to break.

I'll continue to see if I can narrow it down and get back to you when/if I do.
