I have used atlassian, reviewboard and reitveld
(codereview.appspot.com) and prefer the latter. You can host it
yourself, or on you google apps domain or just use
codereview.appspot.com like most projects do. It has a cli tool
(http://codereview.appspot.com/static/upload.py) for uploading
changesets for review from svn/git/hg working copies.
