I have finally seen this problem again since the dynamic log level facility was 
added:

I'm not too familar with NB commands but I think the 5: refers to a buffer 
number?  In this case 5 is the buffer of the incorrect file that the sign 
appears in (should be 8).  There are no buffers 0 or 1, (clewn)_console is 
buffer 2.  Hope this helps.
