No idea how reputable any of these are...
If all you want is a simple poll, my livingcode.org site is running on DreamHost, which offers a polling component. I haven't tried it, but I'd be willing to figure it out and host the poll.
I seem to remember Sourceforge-hosted sites with polls, but I can't find any info on Sourceforge about their support for it.
