I assume that this from logging in with Persona?

Not quite sure how that happened. The grant_type is hard coded so there
must be something that I missed. I'll take another look tomorrow.

Part of me wants to say "at least it's failing in a consistent way and
not 100% of the time" but even if that is true, it sounds like I still
have some PHP spelunking to do :)
