QGIS3:
from PyQt4.QtGui import QComboBox
becomes
from qgis.PyQt.QtWidgets import QComboBox
QgsLayerTreeEmbeddedWidgetRegistry.instance().addProvider(provider)
becomes
QgsGui.layerTreeEmbeddedWidgetRegistry().addProvider(provider)
