I don't see where QGIS Server could differ from QGIS Desktop in retrieving the output CRS. It seems to use the same mechanism (through QgsCRSCache). Andrea, can you confirm it works properly inside QGIS Desktop? Would you share the user defined CRS and some extract of your data in that CRS?
