My suggestion isn't an open source one (but built with OS Components) -
take a look at Fulcrum. Offline data collection and it will do about
anything. I use it in conjunction with QGIS and PostGIS for data
collection. While not free it does make life a tad bit easier in the
data collection realm...and super friendly to qgis
