Hi Anna,

I would start by examining how GDAL reads your shapefile, by executing at the commandline:

  ogrinfo name.shp name -summary

Then follow the error messages there (or paste the response here so we can all see).

Also verify that the shapefile's associated files live in the same directory: .shp .shx .dbf
