Trying a uri Martin pointed me to, I noticed that x and y size were apparently exchanged. 
 However, /some/ WMS servers seem to need a square data window, and some others seem to 
need the "real" aspect ratio.  I added a new option '--square' to force SizeX = 
SizeY.  I also changed the calculation of the rectangle to utm.

The following two calls now give maps which look good at first glance:

# Germany/Nordrhein-Westfalen, 1:5000
./make-gdal-wms.pl --base=http://www.wms.nrw.de/geobasis/adv_dgk5 
--layer=adv_dgk5 --size=0.5 --square
# Portugal, 1:100000
./make-gdal-wms.pl --base=http://mapas.igeo.pt/wms/sc100k --layer=sc100k 
--size=20

Again, any insight would be appreciated...
