I think it could be a problem with the header of the .ecw file. May be the
projection or datum of the ecw file is not reconized by Gdal. I have
experienced problems with maps with the ETRS89 datum.
You can check the datum and projection of the ecw file with gdalinfo:
gdalinfo filename.ecw

I don't know why Gdal (at least wich is installed in my pc) does not open
files with the ETRS89 datum, but I have solved it editing the header of
the ecw file. I have replaced the datum and projection info in the header
with the corresponding EPSG code and it works fine.
example for ETRS89 / UTM 30N --------> EPSG:25830
The only way I know to do this is with the "ECW header editor" utility
supplied by ERDAS and er-mapper. This is a free for use utility but it runs
on Windows. You will found different versions of this utility if you
perform a web search.
