I've used the following setup with success:

(I use courier-imap for most of the e-mail accounts)

#!/bin/sh
logger Creating home directory in $1.
mkdir -m 700 -p $1
cd $1
maildirmake Maildir
chown -R mailuser.vmail $1
---

(The chown is a just-in-case sort of thing)

The /var/qmail/control/dirmaker contains:
/var/qmail/bin/create_homedir

In the LDAP directory, I have:
mailMessageStore: /mail/store/hostname/username/Maildir/
homeDirectory: /mail/store/hostname/username/

Courier-IMAP gets confused if you don't do things that way, I've found.
Anyway, that's what worked for me.
