Is your root path set correctly in your IMAP client? It needs to be
INBOX.  On outlook you can change this on the Advanced tab for your
account.

I've got courier-imap successfully running with qmail-ldap and
auth_imap, but 
there's a problem when creating new toplevel folders: the server always 
responds "Invalid mailbox name".

The FAQ at courier-imap's site tells, that this error occurs in broken 
imap-clients, but i've tried it with several ones (netscape messenger, 
mozilla, kmail, imp and outlook express) and i can't believe every of
these 
clients should be broken in the same way...

Has someone got this running with qmail-ldap ? Can this be a problem
with 
directory structure ?

I've following directory layout:
Maildirs are located in /data/mail/domainname/username/Maildir/

in the rc-file, qmail is started by: qmail-start ./Maildir/

the imapd startup:  
exec_prefix=/usr/local/courier-imap

  exec /usr/local/bin/tcpserver -c 25 -l 0 -v -R -x /etc/tcp.imap.cdb 0
imap \
  ${exec_prefix}/sbin/imaplogin \
  /var/qmail/bin/auth_imap \
  ${exec_prefix}/bin/imapd Maildir/ 2>&1

/var/qmail/control/ldapmessagestore: /data/mail
and a mailmessagestore-attribute in ldap has always following format: 
domain/uid
